Pennsylvania Tax Forms
PA Sales Tax Exemption Form
Hermance Machine Company is required to collect Pennsylvania
Sales Tax for all sales originating or with a destination in
Pennsylvania. If you or your company are tax exempt, this
form is required by Hermance Machine Company as proof to the
taxing authority. Please download, print and complete the
form and fax it back to us at (570) 326-0131. Include all
information requested to avoid delays in the processing of
your order.
PA Request Refund of Sales Tax Paid
If you are tax exempt, and paid sales tax, this form
will allow you to request a refund from the state.
Please note that once Hermance Machine Company has paid
the tax to the state, the refund must be requested from
the state directly. This form must be mailed to Pennsylvania's Department of Revenue
as noted in the form.
New York Tax Forms
NY Sales Tax Exemption Form
Hermance Machine Company is required to collect New York
Sales Tax for all sales originating or with a destination in
New York. If you or your company are tax exempt, this
form is required by Hermance Machine Company as proof to the
taxing authority. Please download, print and complete the

information requested to avoid delays in the processing of

NY Application For
Sales Tax Refund
This form is to be used to claim a refund of New
York Sales Tax if tax has been paid but you later
determine that the purchase was tax exempt.
Credit Application
